Adoption Agreement
NOTE: this page is for adopter's who have been approved to adopt and have been through the adoption process. Do not fill out the below form unless instructed to do so by a representative of SSBR.
The below Adoption Agreement is to be signed before taking home one of SSBR's rescued companions. This agreement is between Southern States Bully Rescue Inc. and the adopter of the animal being released by SSBR. Upon signature and adoption donation being submitted to SSBR, the adopted animal's veterinarian records will be emailed to you. Please understand the adoption donation goes directly to the costs incurred of the Rescue's mission.
